GTA West Chapter Golf Day at Caledon Golf & Country Club - June 17, 2019

For your convenience, if anyone is interested in paying by credit card, kindly contact the OEL Provincial Office at 905-238-1382 to process payment. Kindly follow up with Louie for with your registration details. For those of you paying by cheque, kindly fill in the registration form (link below) with your cheque payment and mail to the address noted on the registration form.
 
We would graciously accept any donation for our prize table as well.
